Privacy & Security Policy

Empowered Performance Physical Therapy LLC is committed to protecting the privacy and security of confidential information, including protected health information (PHI), in accordance with applicable laws including HIPAA.
1. Use and Disclosure of Information
Confidential information will only be used or disclosed for authorized purposes, including treatment, operations, and administrative functions. Information will be limited to the minimum necessary.
2. Access Control
Access to confidential information is restricted to authorized individuals with a legitimate need. Each user will have a unique login and is responsible for maintaining the confidentiality of their credentials.
3. Security Safeguards
The company implements reasonable administrative, physical, and technical safeguards, including:
Use of secure, password-protected systems
Encryption of data in transit and on devices where applicable
Limiting access to authorized users only
Secure storage of physical and electronic records
4. Password & Device Security
All systems must use strong passwords. Default passwords must be changed. Devices used to access confidential information must be secured and, where applicable, encrypted. Multi-factor authentication will be used when available.
5. Workforce Responsibilities
All workforce members must protect confidential information and follow company policies. Access will only be granted after acknowledgment of responsibilities.
6. Training
Workforce members will receive training on privacy and security requirements upon onboarding and annually thereafter. Training completion will be documented.
7. Breach Response
Any suspected or actual breach must be reported immediately. The company will investigate and respond in accordance with applicable laws, including notification requirements.
8. Data Retention & Disposal
Confidential information will be retained and securely destroyed in accordance with applicable laws and best practices to ensure it is not recoverable.
9. Updates to Policy
This policy will be reviewed and updated as necessary to reflect changes in operations or legal requirements.
